Looking through all these posts there is something that I haven't seen mentioned. What I call a "General health" card.

Basically every 6 months, (and before hiking season) I make up a card that outlines important medical information.

On the card I put:
The date the card was printed
My name
My Age
My weight
My blood type
A contact Phone #
A list of important medical information. (i.e Asthmatic - Severe, No known medical allergies, Heart Murmur (VSD))

And anything else that could be important to know quickly( if you are on something that has a high chance of interacting, I would recommend noting that as well). I make sure to laminate it so that it will stand up to some wear and tear.

I print it on neon paper and it sits in front of my Drivers License. When hiking, I have the one in my wallet, and an additional one in the medkit.
